Dear Workers' Compensation Board Representative,

I hope this message finds you well. I am writing to inquire about the process for filing a workers' compensation claim, as I was recently injured while performing my duties at work. I would appreciate your guidance regarding the necessary steps to apply for financial support during my recovery and to access medical treatment through the appropriate channels.

On March 16th, I sustained an injury while working at the company, where I am currently employed as a Autobody Technician. The accident occurred during the course of my regular duties when I slipped on a wet surface in the warehouse and injured my lower back. I reported the incident to my supervisor, Mark Saltun, on the same day, and I have also documented the incident in accordance with our company’s internal procedures. Then, Mark told me I should take the rest of the day off to follow protocols, but I should return the next day unless I do not feel well. 

At present, I am experiencing ongoing discomfort and limited mobility due to the injury, which has made it difficult for me to perform my job. Upon the request of my supervisor, I visited the doctor at a local clinic, and he did prescribe some anti-inflammatory medication to treat the swollen areas of my lower back. Then, he said that I should look into physiotherapy of some sort, but I have not yet sought that kind of medical treatment, as I want to ensure I follow the correct procedures outlined by your board and by my employer’s insurance policy. I am particularly concerned about ensuring that any medical costs I incur are eligible for reimbursement or are directly covered under workers' compensation. 

Given these circumstances, I would like to request your assistance in ensuring that I follow the correct procedures from the outset and that I fulfill all obligations on my part to make this process as efficient as possible. My goal is to recover safely and return to work once I am medically cleared to do so. Please let me know the best way to proceed or if there is a preferred contact person I should follow up with directly. I am happy to provide any additional information or complete any necessary paperwork at your earliest convenience.
